🌿 Medicaid Waiver Resource Center

Medicaid Waiver Programs
That Cover Pest Control

A comprehensive guide to Medicaid Home and Community-Based Services (HCBS) waivers by state — and how pest control is covered as an approved environmental modification or health & safety service.

1915(c) HCBS Waivers Medicaid Managed Care HCBS Final Rule 2014 Environmental Modifications Health & Safety Services

Why Medicaid Waivers Cover Pest Control

Under the federal Medicaid HCBS Final Rule (2014) and Section 1915(c) of the Social Security Act, states may cover pest control as part of environmental modifications, health and safety services, or home modification benefits — when an infestation poses a health or safety risk to a Medicaid waiver recipient.

🏛️

Environmental Mods

HCBS waivers may authorize pest control under "environmental modifications" — physical changes to the home that support the member's health, safety, and ability to remain in the community.

🪐

Health & Safety

Cockroaches trigger asthma, rodents spread disease, and bed bugs cause documented psychological harm. States increasingly recognize pest control as a covered health and safety intervention.

📋

State Plan Amendments

States can add pest control to their 1915(c) waiver approved service lists via State Plan Amendments (SPAs). Indiana, Illinois, Ohio, and Michigan all have pathways for this coverage.

What Is a Medicaid HCBS Waiver?

A Home and Community-Based Services (HCBS) waiver — also called a 1915(c) waiver after the section of the Social Security Act that authorizes them — allows states to use Medicaid funding for services that help people with disabilities, elderly individuals, and those with chronic conditions remain in their homes and communities rather than in nursing facilities.

Each state designs its own HCBS waivers and decides which services to include. Services commonly covered under HCBS waivers include: personal care, home health aide, adult day health, respite, home modifications, and in many states — environmental modification services like pest control when infestation creates a health or safety risk.

The 2014 HCBS Final Rule from CMS (Centers for Medicare & Medicaid Services) strengthened requirements that HCBS waiver settings be home-like, safe, and supportive — which has led more states to authorize environmental health services.

Frequently Asked Questions: Medicaid Waivers & Pest Control

A 1915(c) Home and Community-Based Services waiver is a Medicaid waiver authorized under Section 1915(c) of the Social Security Act. States use these waivers to provide services that help individuals remain in their homes instead of nursing facilities. Many 1915(c) waivers include 'environmental modifications' or 'home modification' benefits — and pest control qualifies when an infestation poses a documented health or safety risk to the Medicaid waiver recipient.

This depends on your state and plan. Bug.Care handles all prior authorization directly with your Medicaid plan or waiver case manager — you don't fill out any paperwork. We submit the necessary documentation, including the health/safety rationale for pest control coverage, on your behalf.

If you're on a waitlist for a 1915(c) HCBS waiver but currently enrolled in standard Medicaid (Medicaid fee-for-service or a Medicaid managed care plan), you may still qualify for Bug.Care services through your current Medicaid enrollment. Contact us to verify — many clients qualify before their waiver is active.

Traditional Medicaid covers standard medical services (doctor visits, hospital stays, prescriptions). A Medicaid Waiver (1915(c)) extends coverage to home and community-based services like personal care, home modifications, and in some states pest control — specifically for people who would otherwise need nursing home care. Bug.Care works with both traditional Medicaid and Medicaid Waiver enrollees.

Bug.Care is a licensed Medicaid provider. We bill your Medicaid plan directly using appropriate HCBS or environmental modification billing codes. You never receive a bill, a claim, or any invoice. The state pays Bug.Care directly — just like any other Medicaid-covered service.

We're expanding rapidly. If your state isn't yet in Bug.Care's service area, you can still use this guide to understand your state's Medicaid waiver coverage options and contact your local Area Agency on Aging, Center for Independent Living, or Medicaid waiver case manager about pest control coverage. We may also be able to provide guidance on how to request pest control authorization from your state's waiver program.
